How to add a custom form to your blog

How do you add a 'contact me' form to a blog? I want to customize the size and color scheme to match the layout of my blog, is that possible?

Two of the most popular form creators are Zoho Creator and Google Docs. Each allows you to drag and drop text boxes, buttons, and any other entry field you can imagine. Both of these services work great if you want to plop a form into the body of a post and link to it from the main page of your blog. If you are looking for a little more control over the size, color scheme, and you don't want a "created using..." logo attached to the form, I highly recommend Formlogix. It does require a paid subscription after 30 days to maintain ad free forms; however, the monthly fee is a reasonable $1.99.

I used Formlogix to create the "Get your questions answered" form on my sidebar. Here are some highlights from my experience with this service:

  • The dimensions of the form, colors, and font are customized to fit my template.
  • No logo is present on the form, you will only see a Formlogix icon if you fill out the form and press submit.
  • At no point does the form direct to another site, it is completely transparent to the user.
  • Submitted forms are automatically emailed to me. I do not have to log in to the Formlogix site if I choose not to.
  • Free for 30 days, then 1.99/month to maintain advertisement free forms.

Update: One of my readers pointed out that unless you upgrade to the 1.99/month subscription, a "powered by" logo and advertisement will appear on the form. I've updated the post to clearly reflect this.

thanks for sharing this great site!
the only remark that i got is that they are 30 days free, not 100%e, and that they are charging 1.99 per month.

Thanks for the comment. I updated the post to clearly indicate that after 30 days some of the features change unless you upgrade to a paid subscription. Good catch, and thanks again!

I have been playing around with blogs lately and think best method I found to make forms is to manually edit html and link to formmail usually provided by hosting company ey. Since it's included in the package it's free kinda.
